Southern Colorado’s Hospice Leader:
We have long set the standard for end-of-life care in Southern Colorado. And we continue to
and address the individual needs of the patients and families we are privileged to serve. As a
nonprofit, we care for everyone who needs us, regardless of ability to pay. We offer grief
support to anyone who has suffered a loss, whether or not their loved one was our patient.
And we provide educational offerings on all matters related to death and dying throughout
the area. Since the beginning of our work in Colorado we have made it our mission to
preserve the dignity of our patients and provide them with a world class level of care.
Please contact us anytime, day or night.

Sharing the Journey, it’s a phrase we use often here at Sangre de Cristo Hospice… we
refer to our patients time with us here as their journey, it is also the name of a book containing
a collection of stories written by patients and their families. And when we decided to produce an
educational TV show about the basics of hospice care we looked at several names and Sharing
the Journey just seemed to fit the bill. We approached our friend Nashville recording artist
Lucas Hoge to put together a theme song for the show and he sent back a upbeat piece that
shared the heart of Sangre de Cristo Hospice. We are excited to announce the release of this
song to the market! Sharing the Journey is available for purchase on itunes
